Dear Team Coordinator,

We are excited that you are participating in Fear Factor 2009 at The University of Findlay.

We ask that you appear, with your team at The University of Findlay Environmental Resource Training Center, located at 5000 Fostoria Avenue, Findlay Ohio, between 8:00 a.m. and 8:15 a.m. so that we may register your team and give you your Fear Factor 2009 Tee shirts, meal tickets for lunch and any last minute instructions.

We require that all participant’s parents (if the participants are under 18) and adult participants (over 18 years of age) sign a waiver and release form.

Please print out the Agreement to Participate Form, have the participants’ parent or legal guardian sign it and turn in the signed form at registration.
