The University of Findlay - Progression Requirements

During P2 Year:

Document completion of 30 hour service project
     -Must be finished with signed documentation by April 15th of P2 year

Document completion of required immunizations
     -Must be completed with form approved by Cosiano Health Center day April 15th of the P2 year

Obtain American Heart Association (AHA) Health Care Providers CPR certification
     -Must be taken no sooner than May 1st of the current year and proof must be submitted to the Assistant/Associate Dean of Internal Affairs by July 1st of the summer before P3 year
     -Current students will be scheduled for this through the College of Pharmacy and will have already paid for the course through their fees.

Document completion of tuberculosis check
     -Must be completed during March 1st-April 15th of the P2 year

White Coat fitting
     -Provide lab coat sizes by April 15th of P2 year

Orientation
     -Mandatory attendance all day on the Friday before classes start

Documentation of successful completion of criminal background check
     -To be completed at the beginning of the P3 year in conjunction with the application for an Ohio Intern License

During P3 Year:

Maintaining Ohio Intern License
-Must be renewed every year prior to the due date

Document completion of tuberculosis check
-Must be completed during March 1st-April 15th of the P3 year

Document completion of a minimum of four (4) hours of academic service learning each semester

During P4 Year:

Maintaining Ohio Intern License
     -Must be renewed every year prior to the due date

Document completion of tuberculosis check
     -Must be completed during March 1st-April 15th of the P3 year. For more information contact Cosiano Health Center

Renew American Heart Association (AHA) Health Care Providers
     -Must be completed during the summer between P4 and P5 years
     - Students will be scheduled for this through the College of Pharmacy and will have already paid for the course through their fees.

During P5 Year:

Maintaining Ohio Intern License
     -Must be renewed every year prior to the due date
     -If any P6 experiential rotations are obtained outside the state of Ohio, a valid intern license from that state must be obtained and proof provided to the College of Pharmacy

Document successful completion of a criminal background check
     -Must be obtained from Verified Credentials (http://myvci.com/findlay) during March of the P5 year

Apply for graduation prior to October

Document completion of required physical exam
     -Must be completed with a form approved by the Cosiano Health Center sometime after March of the P5 year and submitted no later than April 15th of the P5 year

Obtain American Heart Association (AHA) Health Care Providers ACLS CPR certification
     -Students are scheduled for this by the College of Pharmacy during the Spring semester. This is paid for by fees paid during this academic year.
     -If taken elsewhere, proof must be provided that it was taken no sooner than January 1st in the P5 year and proof must be submitted to the Assistant/Associate Dean of Internal Affairs by April 15th of the P5 year
