Incoming Freshmen Application Form

If you have completed at least one semester of full-time enrollment at The University of Findlay or at another accredited college or university, please use this form.
UF or Transfer Student Application Form

Transfer Students: If you have transferred from another accredited college or university, you may request transfer of honors credit taken at your school. This request must be accompanied by appropriate documents including course syllabi and description. The Major Honors Project (HNRS 495) must be completed while at The University of Findlay.

Reapplication Form

If dismissed from the Honors Program, you may reapply to the Program if your cumulative GPA is 3.50 or higher. To reapply, print a copy of the Reapplication Form and complete the top section and sign it. Your advisor's signature is also required. Once completed, give the form to the Honors Program Chair.
