Take I-20, acceptance letter and proof of financial scholarship to the U.S. Embassy or Consulate nearest you in your country of citizenship.  You should call ahead to make an appointment and to inquire about the visa application fee.  Students are responsible to know all the information on page 2 of his/her I-20 form.
