Accommodations and Services

Eligibility
Eligibility is determined through current documentation records of a disability that causes significant impact in the educational setting.  It is the student's responsibility to register each semester with the Office of Disability Services.  Once appropriate paperwork is approved and accommodations are discussed, the Director of Disability Services will initiate services.

What is acceptable documentation?
Acceptable documentation should:    
a. Be current (in most cases, within 3-5 years)
b. Recommend academic accommodations
c. State a specific diagnosis
d. Explain medical and educational history related to ones disability
e. Give level of severity of the disability
f.  List medications/treatment
g. State functional limitations due to the disability

The Office of Disability Services does not provide testing for any disability diagnosis.  Please contact the office for local referral information:
