Photography & Videography Model Release Form
Every person who appears in photographs or video footage used by the University for publicity and marketing purposes must have a signed a model release form on file.
A copy of each form should be saved for liability purposes.
Download and print the attached Model Release Form. Please have the persons shown in your photos or videos sign the form before using the photos or video footage in any format, including print publications or on the website.
