official university name badges are for university faculty and staff only. Special consideration for non-faculty and staff members will be evaluated on a case-by-case basis.
Please allow for four-six weeks from the time you submit your order to receive your new badge.
Name badges will be billed to the cost center provided in the form below. The badges currently cost $9.00+ each.
