All students, faculty and staff wanting to use the student recreation center must first fill out a liability waiver form.
Please print the attached form, fill it out and return it to the Koehler Recreation Complex front desk. Forms are also available at the Koehler Recreation Complex's front desk. This form must be completed each academic year.
