​General Policy Information


All full-time students (9 credit hours for graduate and 12 credit hours for undergraduate), and health professions students without regard to course load*, are required to enroll in the student health insurance plan unless proof of comparable coverage is furnished. 


*The student health insurance requirement applies to students in the following health professions programs without regard to the number of credit hours they are enrolled in: 


Nuclear Medicine Visiting: Certificate; Healthcare Management Major; Health Informatics Certificate; Master of Business Administration – Healthcare Management Concentration Program; Master of Science in Health Informatics Program; Diagnostic Medical Sonography Major; Diagnostic Medical Sonography AA; Diagnostic Medical Sonography Certificate; Echocardiography Major; Echocardiography AA; Echocardiography Certificate; Medical Laboratory Science Major; Nuclear Medicine Technology Major; Nuclear Medicine Technology AA; Nuclear Medicine Technology Certificate; Master of Science in Radiologic Sciences; Exercise Science for Health Professions Major; Health and Physical Education Major; Strength and Conditioning Major; Wellness and Health Promotion Major; Nursing Major; Master of Occupational Therapy Program; Doctor of Occupational Therapy Program; Doctor of Physical Therapy – Weekend Program; Doctor of Physical Therapy – Traditional Program; Master of Physician Assistant Program; Healthcare Management Major; Social Work Major​; Doctor of Pharmacy– On-Campus Pharm.D. ​Program; Doctor of Pharmacy -Distance Pharm.D. In Summer 2024, UF will announce the 2024-25 student health insurance policy, rates & requirements. So long as the University's insurance requirements do not change, students will have an opportunity to submit a waiver request for the 2024-25 academic year. Decisions regarding waiver approval or denial will be based on the student health insurance requirements in place at the time the decision is rendered. ​​

*An important change is being made to the waiver form process. Individuals must meet the four published requirements* or the student's waiver will be automatically denied.  Over the last two academic years, the University has noticed individuals submitting substandard coverage, including policies that do not meet the Affordable Care Act minimum benefit requirements and are not underwritten by a U.S. Domestic insurance carrier. For this reason, insurance policies from the following carriers will be automatically denied: 


Bajaj Allianz, Berkshire Hathaway Travel, Bupa Global, Edelweiss General Insurance, Global Benefits Group (GBG) Limited Insurance, Golden Rule (medical insurance), ICICI Lombard, IMG, ISO – International Student Health Insurance, Molina Healthcare, TATA AIG, Zolve Student Health Insurance.​


*​In order to waive the University of Findlay Student Insurance Plan, and have the related charge removed from their billing statement, ​student's alternate coverage must meet the requirements published on the Student Health Insurance main webpage​.


WHEN ENROLLED IN THE  UNIVERSITY OF FINDLAY STUDENT HEALTH INSURANCE PLAN:
  • You can access telehealth medical and telehealth behavioral no matter where you are located. 
  • ​Your student plan also links you to a national network of providers. If you are living away from campus, please call and we can help you access care in your community. ​
The Preferred Provider for the UHCSR Plan is: UnitedHealthcare Choice Plus.

Insurance Coverage for Intercollegiate Athletes:
 
Intercollegiate athletes are covered by a secondary insurance policy for all athletic related injuries not covered by UHCSR. Inquire within the Athletic Department for more information. Injuries incurred while participating in club or intramural athletic activities are covered by UHCSR.  

*Please note, per NCAA, Athletics requires all student athletes to provide proof of insurance.  This is separate and in addition to the University's student health insurance requirement. As a reminder, Cosiano Health Center is a free clinic for the campus community. The health center can provide general health assessments, numerous diagnostic tests, wound care, first aid, physicals as well as referrals to specialists in our area.
