Students will be evaluated upon completion of each Level I Fieldwork rotation utilizing the following forms:
The UF OT program utilized the AOTA’s Fieldwork Performance Evaluation for all Level II Fieldwork placements. This will be completed through our EXXAT Software program. Please contact the Academic Fieldwork Coordinator if you do not receive the link.
