The
Clinical Site Information Form (CSIF) is used to collect information from clinical sites to facilitate student placements, assess learning opportunities available to students, and provide documentation for accreditation. If you are able to complete it electronically, then you may update it on an annual basis through the Clinical Performance Instrument (CPI) website.
We ask the student to collaborate with you to assist with the completion of this form. If you have already completed your annual update, please notify the student. Feel free to check with the Experiential Educator, if you have other questions.
