Congratulations on the new job!

When you end employment on STEM OPT, you must provide us with the I-983 Training Plan page 5 final evaluation page from that employer. 

When you add new employment, you must provide us with a new I-983 Training Plan completed by you and the new employer and also a new letter from the employer on company letterhead including all of the following: 

Please also include the last day of employment with the original employer and the first day of employment with the new employer. We will need these documents before a new I-20 can be issued with the new employment information.
